2011-2012 Athletic Participation Papers

DEADLINES for 2011-2012 CIPPE FORMS and $10.00 Participation Fee

Fall Sports (Begin August 15th) – Deadline is 3:00pm August 10th
Winter Sports (Begin November 18th) – Deadline is 3:00pm November 15th
Spring Sports (Begin March 5th) – Deadline is 3:00pm Feb 29th

Deadlines will be strictly enforced!

Students will NOT be permitted to practice on the first day if the deadline is missed.
 

A mandatory 24 hour processing period will be enforced for any forms handed in after the start of the season. No students will be able to participate until forms are processed.

 
 
 
Forms needed for participation in the first sport of the school year
   CIPPE Instructions and deadlines
 
 
Forms needed for participation in additional sport seasons
(For those that already played a school sport during this school year)
 
 
Additional Forms needed for wrestlers
 

A $10.00 PARTICIPATION FEE for each sport in a single season MUST accompany the completed physical packet in order to be eligible for sports participation!  CHECKS should be made payable to the Somerset Area School District (SASD).  Please DO NOT send cash payment!
 
Upon completion of all required information, the CIPPE and Somerset athletic forms should be returned to the athletic office to be kept on file. Paperwork may be taken directly to the athletic office or mailed to the athletic department to be processed.
